Although rumors about BOTOX® circulate less frequently than they used to when BOTOX® was first introduced, they still need to be dispelled every so often. Brief History of Breast Augmentation

The desire to increase the size of one's breasts is not a new concept though the popularity of breast augmentation has sky-rocketed in recent years. But as far back as the late 19th century, women were seeking larger, perkier breasts, and paraffin was one of the first materials used to enhance women's breasts. However, paraffin (commonly used in candle making) caused infections and lumps to form in the patient's breast tissue. Finally, in 1920, using paraffin to augment breast was abandoned, and doctors then began enlarging breasts by using fat from other parts of the body. Unfortunately, however, the fat was reabsorbed into the body and left the breasts asymmetrical. Using fat transfers was also abandoned—around the 1940's.

In the 1950's, another method was attempted: polyvinyl sponges were inserted into the breasts, but this was not successful either as the sponges hardened and shrunk, causing infections. The sponges had been linked to cancer in some women.

In 1961, two plastic surgeons created the first silicone breast implant, and then a few years later, the saline implant was developed in France. Since then, improvements have been made in both saline and silicone implants, but in the early 1990's people began to question the safety and efficacy of silicone implants. In 2006, however, the FDA gave silicone breast implants its full support, and now they’re a very popular choice for cosmetic surgeons all over the world.

Scarring After Breast Lift Surgery

Women are naturally concerned about scarring resulting from plastic surgery, which is understandable. When you go out of your way to improve your appearance, you don't want to mar it with scars at the same time. Yet, the nature of any surgery includes an incision of one sort or another. And, incisions used in breast lift procedures, as in any others, tend to leave scars.

The good news is that the techniques developed for breast lift surgery place incisions where they are least likely to be seen. Most scars eventually fade and are minimally visible in most patients. Scarring will be in proportion to the type and extent of incisions, which is determined by the degree of lifting necessary. The greater the amount of lift required means the greater the potential for scarring. Procedures that require less lifting will also require limited incision patterns.

Breast Augmentation and Cancer Screening

A study by the Group Health Cooperative, published in JAMA January 28, 2004 indicated that breast implants appear on mammograms as a white area that obliterates the breast tissue. Unable to view the breast tissue, screeners miss 22% more cancers in women with breast implants than in those without them.

While breast implants do "decrease the ability of mammography to detect breast cancer," the same study also found that the tumors discovered in women with implants were, in fact, smaller than those found in women without implants.

Several theories were presented to explain this:

Women with implants may have less breast tissue making detection of a lump easier.
Tumors might be easier to detect because the implant provides a firm platform against which the tumor would be easier to feel.
Women with breast implants are more diligent in checking their breasts than those without implants.
Being more conscious of their bodies, women with implants may seek medical care if they detect even a small change.

Women with implants are encouraged to have regular mammograms, and to request one if they detect any abnormality. Seek out a facility that is experienced in using the implant displacement view. This technique moves the implant aside to get a clear view of the breast tissue.

Breast Augmentation and Breastfeeding

Breast milk is the perfect baby food, and breast-feeding the ideal way for mother and child to bond. If you plan to breastfeed after having a breast augmentation, be sure you let your cosmetic surgeon know your intent, as it will help him plan implant options that will give you the best outcome.

The action of reaching across the chest, as needed in sports like tennis and golf, can be slightly impacted. The larger your breast implants, the greater the possibility of this concern.
Your arms’ range of motion could be affected in some cases, giving you the feeling that the implants are “in your way,” while exercising. This is usually an early sensation, and you will adapt.
With breast augmentation, you are adding a bit of weight to your breasts, which you may notice in high-impact activities like jogging. A proper exercise support bra can help.
Professional swimmers may experience slightly increased drag in the water.

Types of Breast Augmentation Incisions

The decision regarding where to have your breast augmentation surgery incision placed is one that you and your experienced cosmetic surgeon will make together. Here are the four types of breast augmentation incisions generally used:

Inframammary – This common type of incision is made underneath the breast, where the crease naturally conceals the scar.
Transaxillary – Another popular choice, this type of incision is made within the armpit, meaning there is no scarring of the breast. This option cannot be used with silicone implants.
Periareolar – This type of incision is also widely used. It is made around the nipple, along the lower edge of the areola. The tiny scar is easily concealed in this highly pigmented area of skin. If you are having a breast lift along with your breast augmentation, this is often a preferred choice.

Breast Augmentation Recovery Time

Recovery from breast augmentation surgery will be different for every patient. But in general, this type of surgery requires four to six weeks for complete recovery. Here are a few other points to keep in mind:

• A majority of breast augmentation patients are able to perform light activities the day after surgery and can return to driving within four days.
• Your breast augmentation cosmetic surgeon will instruct you on when you can remove bandages, but usually that is within several days.
• You should be able to return to work within one week.
• You should refrain from strenuous activity or heavy lifting for four to six weeks after the surgery.
• Discomfort and soreness is to be expected, especially in the first and second week, and prescription pain medication can help with that. You may also be prescribed an antibiotic.
• Some bruising and swelling is also normal.

What to Expect During a Breast Augmentation Procedure

In most cases, breast augmentation surgery is performed as an outpatient procedure, which does not require an overnight stay in a hospital. General anesthesia s typically used, so you feel no pain during the procedure. The surgery itself lasts from one to two hours.

Here’s a brief outline of what happens during the surgery:
• Small incisions are made, and implants are inserted either on top or behind the pectoral muscle. (Read more about the types of incisions used.)
• If your implants are saline-based, your surgeon will fill the implants at this point.
• The incisions are sealed with sutures that your body will eventually absorb.
• Once the surgery is complete, you will be monitored in a separate recovery area, staffed with qualified nurses.

Study Determines Most Popular Breast Implant Size

A study reported in a recent issue (March/April 2009) of Aesthetic Surgery Journal has found that more than 80 percent of breast augmentation patients choose a size C-cup implant (or 300 to 400cc) size. The study surveyed only those breast augmentation surgeries that were done for only cosmetic purposes.

Study Shows No Cancer Risk for Breast Implants

When the FDA announced its temporary ban on silicone breast implants for breast augmentation, one of the concerns was a possible connection between silicone breast implants and breast cancer. Of course, one reason why the implants were re-approved was that no connection was ever established. Now, a new study gives even more reason to believe in the safety of silicone breast implants. The study shows there is no connection between silicone breast implants and any type of cancer.

The study was published in the International Journal of Cancer's January issue. It was one of the largest and longest every published, following 3486 Swedish and 2736 Danish women who received had received breast augmentation between 1965 and 1993 from the date of their breast augmentation through 2002. The cancer rates for these women were determined and compared to nationwide cancer registries. On average, women in the study were followed for 16.6 years, and some were tracked for 37 years, the entire length of the study from 1965 to 2002.

The study's results showed no significant increase in any cancer due to the women's breast implants. The women actually had lower rates of breast cancer, experiencing only 73% as many cancers as the general population. The study's authors did not present an explanation for this lower incidence. Lung cancer was the only type of cancer that occurred among the women more frequently than in the general population, which the study's authors attributed to a high number of smokers among the study population.

An article written by the American Society of Plastic Surgeons (ASPS) describes the rapid growth in popularity of cosmetic surgery procedures known collectively as “Mommy Makeovers”. This article says that statistics show an 11% rise in mommy makeovers between 2005 and 2006–more than five times the increase of all cosmetic surgery procedures.

Mommy makeover procedures generally include breast augmentation and lift, and tummy tuck. But other procedures may be added, such as liposuction or lower body lift. Between 2000 and 2007 there was nearly 50% increase in the number of men who had abdominoplasty, or tummy tuck surgery. Mini-Abdominoplasty

Body contouring includes a group of several procedures to reshape the abdomen, breasts, upper arms, thighs, buttocks and hips. These procedures are intended to remove fat and excess skin caused by rapid weight loss, pregnancy, loss of skin elasticity, or aging. Mini-abdominoplasty, a modification of the full abdominoplasty is one of these body contouring procedures.

Breast Augmentation for Asymmetry

Maybe. Since breast asymmetry can have different causes, and manifest in different ways, there’s no clear yes or no answer to this question. Asymmetry that is limited to a difference in breast size might be fully corrected with augmentation, reduction, or lift procedures. But many patients (men included) have breasts that need more than one type of correction. Congenital abnormalities, variations in growth, or differences due to disease, such as cancer will determine, to a large extent, the exact corrective procedures needed.

Breasts can be tuberous or conical, one of each shape, one larger than the other and both too small or too large. One or both may sag, or nipple placement may be uneven. One common indicator for surgery to correct asymmetry is a difference of one cup-size or greater between breasts. Others include obvious differences in shape from conical to tubular that affect one’s appearance, or their emotional or psychological status.

The need for correction of breast asymmetry can be classified as cosmetic or reconstruction depending upon the degree of difference, or whether there is significant deformity.

Hmmm, she’s just told you she wants breast augmentation surgery. Don’t underestimate how important it is for the two of you to talk about this and how this affects both of you. As the guy in her life, helping her do some research before her consultation with her cosmetic surgeon will keep both of you well informed and ready to make choices that will satisfy both of you.

Be careful not to rely on what friends say, even if they have had the procedure done, because each procedure is as individual as the patient herself. Attend seminars and research the Internet, especially the American Society of Plastic Surgeons (ASPS) for reliable information about breast augmentation. Breast Implants: When Size Matters

Yes, it’s true. Size really does matter when you’re having a breast augmentation. In fact, it’s one of the biggest decisions you have to make. One study shows that of the women who underwent re-operation, 34% did so to have a larger size implant. On the other hand, smaller breasts, with a fuller lower pole, balanced with your body size and type, are trendier today. It’s well worth the effort to choose the most flattering size the first time.

Silicone or saline; teardrop or round shaped; high or low profile; sub muscular or sub glandular–how does one choose? One factor is the amount of breast tissue, another the thickness of your skin. Breast Augmentation Poses Small Risk

Any type of surgery carries some risk, but is complication-free most of the time. The same thing can be said about the risks with breast augmentation. While the risks are extremely rare, we believe it is in your best interest to understand all there is to know about these risks with whatever procedure you decide to undergo with Dr. Angelchik.

Some of the risks involved with breast augmentation include:

Infection – Dr. Angelchik will give you antibiotics as part of your post-op recovery. If you take them as prescribed, your chances of infection are virtually zero percent.
Anesthesia – Using sedatives or anesthesia is a must with breast augmentation. The risk with anesthesia usually comes in the form of an allergic reaction. If this happens, Dr. Angelchik and his staff will deal with it immediately.
Loss or change in sensation – Some patients may lose sensitivity in their breasts or nipples for a short amount of time after surgery. This may only last six to twelve weeks. However, in very rare instances, this loss is permanent.
Scarring – Scarring is inevitable with breast augmentation. However, these scars will fade after about a year to be hardly noticeable. Some patients who are prone to excessive scarring need to keep this in mind when considering breast augmentation.
Rippling – Saline implants are more prone to rippling than are silicone implants. Those placed behind the muscle, or submuscularly, are less likely to show rippling than those placed in front of the muscle.
Deflation or leaking – If the implant’s shell fails, it can cause leaking of the saline or silicone. Saline is part of the body’s natural makeup, so if your saline implant leaks, it is absorbed into the body. Silicone will stay in the area of the breast and the entire implant will have to be replaced.
Capsular contracture – This is a tightening around the implant caused by scarring that can be uncomfortable or painful. Massaging the implant and taking vitamin E supplement should minimize this effect. Some patients may require corrective surgery if the tightening becomes too much.

Gynecomastia: Male Breast Reduction Surgery

Gynecomastia is an overgrowth of tissue in the male breast. This is different from excessive fat tissue caused by being overweight. The extra tissue is a combination of fat and glandular tissue. The most common cause in younger men is hormonal changes during puberty. In most cases, the condition resolves itself over a two year period without treatment. In other cases, surgical treatment may be necessary.

Gynecomastia may also be caused by:

Benign hormonal changes in the elderly
